Injectable VIP Program Membership Agreement
OpalGlow Medspa
This Membership Agreement (“Agreement”) is entered into as of the Effective Date written below by and between OpalGlow Medspa, located at 655 Amboy Ave Ste 403 Woodbridge, NJ 07095 (“Company”), and the individual signing below (“Member” or “You”)
1. Membership Overview
By enrolling in the Injectable VIP Program, you agree to pay a monthly membership fee of Ninety-Nine Dollars ($99.00).
This program is designed as a maintenance and access program for clients who receive injectable treatments on an ongoing basis. It provides priority access, preferred pricing on select injectable and provider directed aesthetic treatments, and a structured way to plan ongoing care.
Membership requires a six (6) month minimum commitment and automatically renews on a month-to-month basis following completion of the initial term unless canceled in accordance with this Agreement.
Enrollment may be limited to maintain appointment availability and quality of care.
2. Member Benefits
While actively enrolled, members receive the following benefits, subject to provider availability and clinical appropriateness:
Monthly Patient Wallet
Members bank $99 per month into a personal patient wallet while enrolled.
Wallet balances accrue monthly while membership remains active
Wallet funds are non-refundable, non-transferable, and forfeited upon cancellation
Wallet funds hold no cash value
Preferred Pricing on Select Treatments
Members receive preferred pricing on select injectable and provider directed aesthetic treatments, including Botox, dermal fillers, PRP/PRF, and chemical peels, while actively enrolled in the Injectable VIP Program.
Preferred pricing is available in-office only
Member pricing is not published online
Preferred pricing is subject to clinical appropriateness and provider discretion
Preferred pricing may not be combined with promotions or third-party programs
Complimentary Sunscreen at Enrollment
Members receive one complimentary medical-grade sunscreen at enrollment.

Annual Membership Appreciation Gift
As a courtesy for continued membership, members receive a provider selected medical grade skincare product at their annual membership anniversary.
Provided once per membership year
No cash value
Non-transferable
In-Office Skincare Courtesy
Members receive 15% off medical-grade skincare purchases made in office while actively enrolled.
Applies to in-office purchases only
Cannot be combined with promotions or special offers
Excludes memberships and treatment services
Priority Scheduling
Members receive priority access to appointment availability, including morning and limited availability time slots. Appointment times are not guaranteed and are subject to provider availability.
Complimentary Botox Adjustment Window
When clinically appropriate, members receive a minor Botox adjustment within 10–14 days following treatment, at the discretion of the treating provider.
Price Protection
Members are protected from Botox price increases while actively enrolled.
3. Use of Wallet Funds
Wallet funds may be applied only toward eligible injectable and treatment services.
Wallet funds may not be used for:
Retail skincare products
Membership dues or fees
Training or model appointments
Non-injectable services
Wallet funds are non-refundable, non-transferable, and hold no cash value.
4. Payment Terms
You authorize OpalGlow Medspa to charge the card on file $99.00 per month for membership dues.
Membership dues are required to access benefits
Payments are non-refundable
If a payment is declined, benefits will be suspended until the account is brought current
5. Cancellation & Termination
Initial Commitment
Membership requires a six (6) month minimum commitment. Early cancellation results in:
Immediate termination of membership benefits
Forfeiture of all unused wallet funds and promotional benefits
After Initial Term
After the initial term, membership may be canceled with thirty (30) days’ written notice sent to:

Company Termination
The Company reserves the right to suspend or terminate membership for non-payment, policy violations, or conduct deemed inappropriate.
6. General Terms
Membership benefits are subject to provider availability and clinical appropriateness
Specific appointment times, providers, and services are not guaranteed
Membership benefits are for personal use only and may not be transferred
Membership benefits may not be combined with third-party programs (e.g., Alle, Aspire, Evolus)
Member agrees to comply with all current and future OpalGlow Medspa policies
7. Policy Updates
OpalGlow Medspa reserves the right to modify membership terms, benefits, or pricing with written notice in accordance with applicable laws.
8. Acceptance
By signing below, you acknowledge that you have read, understood, and agree to all terms and conditions of the Injectable VIP Program Membership Agreement. You understand that this Agreement is legally binding.
