OpalGlow Membership Program Agreement
OpalGlow Medspa
This Membership Agreement (“Agreement”) is entered into between OpalGlow Medspa, located at 655 Amboy Ave Ste 403, Woodbridge, NJ 07095 (“Company”), and the individual enrolling in a membership (“Member” or “You”). The Effective Date is the date shown on your enrollment confirmation.
1. Membership Overview
This Agreement covers three OpalGlow membership options. Your Selected Plan is the plan named on your enrollment confirmation and payment receipt. You are charged for, and receive the benefits of, only that Selected Plan.
OpalGlow Bank: $99.00 per month, with the full monthly payment credited to your patient wallet.
Monthly Glow: $179.00 per month, including one Signature DiamondGlow treatment credit with each successful full monthly payment.
Year of Glow Pass: $900.00 paid once, including six prepaid Signature DiamondGlow treatment credits and 12 months of pass benefits.
OpalGlow Bank and Monthly Glow each have a six (6) month initial commitment and continue month to month afterward until canceled. The Year of Glow Pass ends after 12 months and does not renew automatically.
2. Member Benefits
Benefits apply while your Selected Plan is active and in good standing, subject to the treatment and pricing rules in this Agreement.
OpalGlow Bank
The full $99.00 from each successful monthly payment is added to your personal patient wallet.
10% off regular priced Botox, individual Signature DiamondGlow treatments, individual SkinPen treatments, and individual chemical peels.
5% off regular priced individual dermal filler syringes.
10% off medical grade skincare purchased in office.
Monthly Glow
One Signature DiamondGlow treatment credit with each successful full $179.00 monthly payment.
Additional Signature DiamondGlow treatments for $179.00 each while the membership is active.
10% off regular priced Botox, individual SkinPen treatments, and individual chemical peels.
5% off regular priced individual dermal filler syringes.
10% off medical grade skincare purchased in office.
Year of Glow Pass
Six prepaid Signature DiamondGlow treatment credits issued after the $900.00 purchase is successfully processed.
Additional Signature DiamondGlow treatments for $149.00 each during the active 12-month pass.
10% off regular priced Botox, individual SkinPen treatments, and individual chemical peels.
5% off regular priced individual dermal filler syringes.
10% off medical grade skincare purchased in office during the active pass.
Percentage discounts apply to the then-current regular price of eligible items. Treatment packages retain their package price and do not receive an additional membership percentage discount.
3. Use of Wallet Funds and Treatment Credits
OpalGlow Bank wallet funds: Each successful full $99.00 monthly payment adds $99.00 in dollar credit to your patient wallet. If an enrollment payment is prorated, its wallet credit is prorated accordingly. Wallet funds may be used toward eligible aesthetic services, including Botox, fillers, DiamondGlow, SkinPen, chemical peels, PRP/PRF, and eligible treatment packages. Wallet funds may pay the remaining balance after an applicable member discount is calculated.
Banked funds cannot be used for membership payments, gift cards, retail skincare, tips, medical weight-loss services, or services not designated as eligible by OpalGlow.
DiamondGlow treatment credits: Monthly Glow issues one credit with each successful full monthly payment. A prorated enrollment payment does not issue a treatment credit; the first credit is issued with the first full monthly payment. The Year of Glow Pass issues six treatment credits after its one-time payment. One credit is redeemed for one Signature DiamondGlow treatment. Dermaplaning, chemical peels, and other enhancements are separate unless expressly included in the booked service.
Unused paid wallet funds and earned or purchased DiamondGlow credits remain available after membership benefits end. They are personal to the Member and cannot be transferred or redeemed for cash except as required by law. Ending a membership does not erase these paid balances or treatment credits.
4. Payment Terms
By enrolling, you authorize OpalGlow to charge your payment method according to your Selected Plan:
OpalGlow Bank: $99.00 on each monthly billing date until canceled.
Monthly Glow: $179.00 on each monthly billing date until canceled. This payment provides a treatment credit; it is not also banked as a dollar credit.
Year of Glow Pass: One charge of $900.00. There are no monthly pass charges and no automatic renewal.
If a monthly payment fails, new credits will not be issued and active member pricing may be suspended until payment is resolved. Credits earned through payments already made remain available. Refund requests are handled under applicable law and OpalGlow’s disclosed policies.
5. Cancellation and Termination
OpalGlow Bank and Monthly Glow: Both plans have a six-month initial commitment. If you cancel before completing it, a $50.00 early cancellation fee applies. After the initial commitment, the plan continues month to month, and you may cancel without an early cancellation fee.
You may request cancellation through your Aesthetic Record patient portal or by emailing customerservice@opalglowmedspa.com before your next scheduled billing date. Cancellation ordinarily ends active benefits at the close of the current paid billing period and stops future monthly charges. Paid wallet funds and treatment credits remain available as described in Section 3.
Year of Glow Pass: Active pass benefits end 12 months after the Effective Date. The pass does not renew, so no cancellation notice is needed to prevent another membership charge. Ending active pass benefits early does not erase unused prepaid DiamondGlow credits.
OpalGlow may suspend or terminate active benefits for nonpayment, serious policy violations, or inappropriate conduct. Previously paid wallet funds and treatment credits remain available or will be resolved as required by applicable law.
6. General Terms
Treatments are subject to provider assessment, clinical appropriateness, and appointment availability.
Membership does not guarantee a specific appointment time, provider, treatment, or result.
Benefits are for the Member’s personal use and cannot be transferred.
Membership discounts do not combine with other OpalGlow promotions or additional discounts on treatment packages.
Banked wallet dollars are a payment method and may be applied after an eligible membership discount.
OpalGlow’s separately disclosed appointment cancellation, treatment, and practice policies continue to apply.
7. Policy Updates
OpalGlow may update terms offered to future members and may change the regular prices of individual treatments. Active percentage discounts apply to the then-current regular prices; membership does not lock a regular Botox or other treatment price.
Changes to a monthly plan’s membership fee or stated benefits will not take effect during its initial six-month commitment without the Member’s agreement. Stated pass benefits remain in effect during an active, paid Year of Glow Pass. Any changes are subject to required notice and applicable law. Changes will not remove wallet funds or treatment credits already paid for.
8. Acceptance
By enrolling in your Selected Plan and signing this Agreement, you acknowledge that you have read and agree to the terms that apply to that plan and the shared terms above. You authorize the payment schedule stated for your Selected Plan and understand that this Agreement is legally binding.

Member: As identified on your enrollment record.
Effective Date: As shown on your enrollment confirmation.
